Assignment preparation

  • You are expected to write this vignette as an essay (therefore you need to write an introduction, body of the essay and conclusion). Sentences and paragraphs should be clearly organised with minimal abbreviation and limited nursing jargon. Where abbreviations are used please place the words in full followed by the abbreviation the first time it is used, for example, three times a day (TDS).
  • Submit assignment in a minimum 12 font size (Arial or Calibri) with 1.5/double spacing.
  • Referencing must conform to the APA 6th edition referencing style guidelines.
  • Reference list will not count towards the total word count.

Assignment instructions

Mr James Walker has just been transferred to the high acuity ward from the Emergency Department because of overcrowding and bed management issues.  He is awaiting a review from the Surgical Team. You have been on the afternoon shift caring for Mr Walker and are now required to provide a comprehensive handover of Mr Walker to the Multi disciplinary team (MDT) (for the  purposes of this paper, this rapport will be much more comprehensive than in a real situation).

Your assignment will be a documentation of this handover, and must include:

  • A brief background and history of the patient.
  • Your current assessment of Mr Walker on the high acuity ward. 
  • You may consider going through primary and secondary survey.
  • Choose to focus on one high priority condition and provide an in-depth discussion of the pathophysiology along with the signs and symptoms, including pharmacology. You need to make clear links to Mr Walker, the case study patient.
  • Identify interventions with rationales.
  • Identify possible complications. How would you assess for, prevent and/or respond to possible complications? How would you manage these and evaluate Mr Walker's progress?
  • Critique the management of Mr Walker. Were there any missed assessments? Has he received the correct pharmacological management during admission?

He fell on the left side of his left arm, and his left arm twisted under his body and left temple area hit very hard on the rock.
