A patient arrived with complaints of continuous dizziness. He was suffering from severe hypertension which was progressing towards the risk of organ failure.

Assessment

I took his vital signs which showed elevated blood pressure of 180/110mmHg and heart rate was above average. I did ECG to confirm that the heart has not suffered any damage. He showed agitated behavior and confusion. The onset of severe untreated hypertension was basically due to the lack of knowledge about the disease and thus, non serious attitude towards its management. I suggested end stage hypertension leading to adverse change.

Planning and Implementation

The consultant advised vasodilators to manage the hypertensive crisis. I administered Nitroprusside IV to the patient. The plan was to enlighten the patient through nursing interventions about the disease and its risks to organ failure. His diet was managed by eliminating caffeine, salty food and saturated fats. I reinforced the importance of following the treatment regimen and visiting the hospital for follow-up appointments. Frequent changes in position and exercises were also advised.

Evaluation:

Patient care protocol was documented. Patient in follow-up visits was found to have normal blood pressure and claimed of feeling much better. He had maintained a proper treatment regimen and had improved his physical health.

Situation:

 I witnessed a case where an individual in a self-contained unit within a long-term care facility was admitted to the hospital due to dehydration and diarrhea. He was infected with Norovirus.

Task:  Such patients are isolated due to the spread of virus through touch and sneezing. The hospital was informed beforehand to implement contact and droplet precautions.

Action:

 I worked under my instructor to follow the standard and additional precautions of infection control.

 Patient was transferred to an isolated ensuite and his door was marked 

I placed soaps and cleaning materials outside the door along with sterile masks, and eye protection.

 I encouraged other staff members and visitors to follow the hand hygiene protocol and to wear proper PPE. 

 Baskets were placed to dispose off the linen and garbage. 

I briefed the family members on the necessary precautions to minimize the spread of the disease.
